A Design Science Approach to Lean-Based Workflow Optimization in Radiotherapy Using Visual Management Dashboards

Radiotherapy departments face increasing pressure due to rising patient volumes, workforce shortages, and complex multidisciplinary workflows. Although Hospital Information Systems (HIS) capture time‑stamped workflow data, these data are rarely transformed into actionable insights that support continuous process improvement. This study investigates how HIS data can be analysed and visualised to support Lean‑based workflow optimisation within radiotherapy. Using Design Science Research methodology, we integrate a literature analysis on Lean in radiotherapy, a stakeholder survey on visualisation interpretability (N=52), and two dashboard artefacts: a real‑time operational dashboard for daily prioritisation and a retrospective performance dashboard supporting continuous improvement. The results demonstrate that combining Lean‑aligned performance indicatorswith cognitively accessible visualisations enables earlier bottleneck detection and improved workflow transparency. The study contributes a transferable design framework for data‑driven workflow optimisation in complex healthcare settings.
