Adolescents with chronic conditions have to learn to self-manage their health in preparation for transitioning to adult care. Nurses often struggle with how to approach youth with chronic conditions successfully. Little is known about the preferences and attitudes of these young people themselves.

In de spreekkamer van de kinderspecialist moeten jongeren leren zelf het woord te voeren. Dat maakt de overgang naar een specialist voor volwassenen een stuk makkelijker.

In het project ‘Op Eigen Benen’ wordt onderzocht wat jongeren met chronische aandoeningen in de leeftijd 12-19 jaar willen en kunnen in de zorg.
